      기본증명서, 결혼하셨으면 혼인관계증명서, 유학생이시면 그동안 I-20모두다… 그 밖에 여러 가지…

      1. Birth Certificates

      6. Passport(s): ALL the pages (including blank pages) of current and expired passports
      7. Approvals: previous immigration documents including prior USCIS approval notices, for example: Form(s) I-797, Form IAP 66 or Form I-20, and/or any Employment Authorization Document, if applicable
      8. Medical Exams: in a sealed envelope
      9. Photographs: submit six (6) passport (full front) style photographs for adults and four (4) for children under 14 years of age

      1. Birth Certificates
      You will need to provide a photocopy of your birth certificate and a photocopy of the birth certificate of each family member applying with you.

      Birth documentation must have been registered within one year after the birth and must comply with the State Department Fees and Reciprocity tables, which specifies documents required and available from each country. Please click on the following link to view the Reciprocity Table and select your country of birth from the drop down menu (Information listed by country and provided by the U.S. Dept. of State.)

      3. PROOF OF DIVORCE OR DEATH
      If applicable, please send us photocopies of divorce or death decrees issued in connection with any prior marriages for you or your spouse.

      4. TRANSLATIONS
      You may translate the document(s) yourself, but a third party (non-family member) must certify that the translation is complete and accurate, and that s/he is a familiar with both the foreign language and with English (see sample certification language below).

      8. Medical Exams:

      Please obtain the medical examination results (in a sealed envelope with your name clearly written on the front) from the provider you select and send the original exam results to our office without opening the sealed envelope. Please ask the provider to also give you a copy of the results of the exam for your records.

      Note: Starting Jan. 1, 2012, civil surgeons MUST use the current form dated 10/11/11, or face rejection by USCIS, requiring the applicant to return to the civil surgeon and resubmit the current form. Please ensure that the correct form is used when you appear for your examination.
