I-693 requires vaccinations, TB test, and syphilis testing.
1) Vaccinations
If you are 18-64, only vaccinations you need are Tdap, MMR, and Varicella. (According to CDC and USCIS, sourced from page 16 of the following pdf document (http://www.cdc.gov/immigrantrefugeehealth/pdf/2009-vaccination-technical-instructions.pdf)).
If you had Tdap within last 10 year, you are good to go.
You need to have two does of MMR from your birth.
Finally, if you had chicken pox and are able to explain what have happened, you do not have to get varicella vaccination.
Do not go to a civil surgeon if you haven’t had these vaccinations listed above. Go to local health dept. and they will charge you 5 to 10 bucks for each of the vaccinations listed above. Civil surgeon will generally charge you 35 bucks or higher per vaccination.
Also, if you have vaccination records from Korea. I would recommend taking a visit to local health dept. anyway because they will usually give you a new American record for free of charge. This is recommended because some civil surgeon will try to milk you for money by not accepting foreign vaccination records.
2)TB test
If you have had TB testing done within last three years and your test result was negative. You are good to go.
Otherwise, go to local health dept. for the test. I was charged only five dollars for it.
3)Syphilis
Contrary to popular belief, you do not need to get tested for HIV anymore for immigration purposes (According to USCIS and CDC, sourced from http://www.cdc.gov/immigrantrefugeehealth/exams/ti/hiv-guidance-panel-civil.html).
Also, USCIS and CDC require syphilis testing to be done in VDRL or RPR equivalent. VDRL takes about 2 weeks but RPR only takes three days.
If you are not short for time, go to local health dept. and get free HIV testing or STD testing. They are usually free of charge. However, most local health depts are likely to use VDRL.
If you are in a hurry, pay a visit to your primary care physician and get RPR. You should be able to bill it to your insurance.
After you have all your vaccination record, TB testing result, and syphilis testing, go to this website to look for Civil Surgeon https://egov.uscis.gov/crisgwi/go?action=offices.type&OfficeLocator.office_type=CIV.
Contact civil surgeons in your area, tell them that you do NOT need any vaccination, TB testing, or syphilis testing, if you do so, you are likely to get charged less than 100 bucks per person and will be out of his/her office in less than an hour.
For me, I-693 preparation bill as as follows:
TB testing ($5.00 from local health dept.)
Tdap+MMR ($10.00+10.00 from local health dept.)
Free HIV testing for syphilis testing ($0.00)
I-693 preparation by a civil surgeon ($53.00)
