LIST OF REQUIRED DOCUMENTS FOR EMPLOYEE
PLEASE SEND US COPIES OF THE FOLLOWING DOCUMENTS. If you are missing some documents, send us copies of what you have and submit the rest as soon as you obtain it. WE DO NOT WANT OR NEED THE ORIGINALS.
1. Copy of EVERY page of your passport AND I-94 (including any Border Crossing card);
2. Copy of your birth certificate with English translation;
3. If you are married, provide us with a copy of your marriage certificate with English translation;
4. If you have been divorced, provide us with a copy of all divorce certificates with English translation.
5. Diplomas, transcripts, certificates. (With English translations)
6. Resume covering the last 5 years of employment experience. (Resume should list employer; dates of employment; job title, job description; hours worked and skills used).
7. Copy of any employment authorization card or employment approval notice (e.g. OPT, EAD, etc.)
8. Copies of every I-20 issued
9. Four copies of your driver’s license in color, slightly enlarged
10. Copy of any credential evaluation received
11. Copy of work experience letters, including job offer letters and employment confirmation letters. Note: these letters must have start date (month/day/year and end date, job title, and job duties. The letter must be on letterhead, dated and signed.
12. If on H1B; copy of current Approval Notice and all prior H1B Approval Notices.
13. Copy of social security card
14. List of present and past membership affiliations with organizations. Include dates (to and from), names and nature of organization.
15. Copy of any licenses you may have, or any certifications that assist in your profession

3. Copy of birth certificate for US citizen children
4. If you have been divorced, provide us with a copy of all divorce certificates with English translation.
5. Four copies of driver’s license, slightly enlarged and in color (if any)
6. Copy of I-20’s issued for dependents (e.g. listing dependent names)
7. Copy of social security card, if any
