2. One to three years at a time forever till the GC decision is made
3. Same as all docs for an H1-B filing PLUS a proof of the LC filing or I-140 approval
4. Can be filed thru a premium processing with an extra fee
5. Time recapture of unused H1-B. Yes, you can add them up or can intentionally use it.
