Case History에서

2)On April 10, We sent a response to your request for us to reveiw an administrative error on your case, referral number OVPO23 xxxx.
3)On April 6, — was assigned to an officer for response.

도대채 administrative error on my case라고하는데 어떻해 해석해야하는건가요? ㅠㅠ
